These templates are designed to be used by anyone and everyone...it's why we are sharing Microsoft Word versions of the forms. Feel free to steal these, customize with your own logos, edit as needed, and send out to folks. Make sure you input your own clinic name and customize with your logo.
The more forms we get, the more we will upload here for public consumption. If you have a form that you like or that you want to see, let us know in the comments section.
This form is especially good for those starting a practice and thinking through their financial policies as well as general consent for care and treatment that folks tend to need.
We'll have more specialized forms but this was one we had handy.
